2. VO2 Max: The Ultimate Predictor of Lifespan

If I had to choose just one metric of heart and vascular health to keep an eye on, I would choose VO2 max without a second thought. This is the maximum volume of oxygen your body can consume per minute of intense work. Essentially, it is an assessment of how harmoniously your lungs, heart, blood vessels, and mitochondria (our cellular power plants) work together during physical exertion.

Weak mitochondria mean chronic systemic inflammation, low daily energy, and rapid wear and tear of the body. Renowned longevity doctor Peter Attia calls VO2 max the single most important predictor of lifespan. A study in JAMA Network Open showed that people with an elite VO2 max have a 5-fold (500%!) reduction in all-cause mortality compared to the low-fitness group. To put that in perspective, having a poor VO2 max is a greater risk factor for an early grave than smoking, coronary artery disease, or diabetes combined.

Think of your VO2 max as a physical savings account for your older years. It naturally declines by about 10% per decade after the age of 30. How to measure it:

  • The Gold Standard: Go to a sports lab for a cardiopulmonary exercise test (CPET). There, they will put you on a treadmill or stationary bike, strap a rubber mask to your face, and record every single inhale and exhale as they ramp up the resistance to your absolute limit.
  • The DIY Cooper Test (12 minutes): Find a running track or a flat, open path. Run or walk as fast as you can handle for exactly 12 minutes. Note the total distance in meters and calculate the result using this simple formula: VO2 Max = (Distance in meters – 504.9) / 44.73
  • Smartwatches: Apple Watch, Garmin, WHOOP, and other wearable devices can estimate VO2 max based on your heart rate and pace during outdoor walks or runs. There is some margin of error, but for tracking your progress over months and years, it is an ideal, friction-free option.

3. Walking Speed

Geriatricians (doctors who work with older adults) call walking speed the “sixth vital sign” alongside heart rate, temperature, and blood pressure. Walking seems incredibly simple to us because we do it on autopilot. In reality, it is a highly complex neurological feat. Your brain, spinal cord, peripheral nerves, vestibular system, heart, and muscles must work as a perfectly synchronized orchestra just to take one step forward without falling. Changes in any of these systems may affect gait speed, balance, and confidence while walking.

Measure out exactly 4 meters in a quiet hallway or on a flat sidewalk. Mark the start and finish lines. Walk this distance at your normal, comfortable pace—not rushing, just your everyday walk—and time yourself with a stopwatch. Divide 4 meters by the seconds it took. That is your speed in meters per second (m/s).

The norm for a healthy adult is between 1.2 and 1.4 m/s. Anything below 0.8 m/s is a reason to be concerned. It is a clear marker of hidden muscle weakness, high fall risks, and even early cognitive decline. A massive analysis of 34,000 older adults in the JAMA journal showed that every extra 0.1 m/s in walking speed reduces the risk of death by 12%.

Interestingly, a slow step is often noticed several years before a person starts having obvious memory problems. If your brain has to expend massive cognitive energy just to keep you balanced and moving forward, it has fewer resources left for memory and thinking.

How to Boost These Three Metrics?

1. Getting Hand Strength Back

  • Farmer’s walk: Grab a pair of heavy dumbbells or kettlebells (aim for about 25% of your body weight in each hand to start) and just walk with them around the room or gym for 30–60 seconds. Keep your posture tall and shoulders packed. Do 3–4 sets.
  • Dead hang: Just hang from a pull-up bar for as long as you can. This not only builds a crushing grip but also decompresses your spine and opens up your shoulders. Our goal is to accumulate a total of 2 minutes of hang time throughout the day.
  • Progressive overload: Don’t get stuck using the same light weights forever. Gradually pick up heavier objects so your forearm muscles and tendons are forced to adapt and grow stronger.

2. Raising VO2 Max

  • The Base (Zone 2 Cardio): Dedicate 150–180 minutes a week to easy running, cycling, rowing, or brisk incline walking. The pace should be comfortable enough that you can speak in full sentences, but just hard enough that you wouldn’t want to sing. This type of training can improve aerobic efficiency and endurance over time.
  • 4×4 Intervals (The Norwegian Protocol): Once a week, give yourself a tough, lung-burning workout. Do 4 minutes of high-intensity work (aiming for a heart rate at 85–95% of your maximum), followed by 3 minutes of very light, active recovery. Repeat this cycle 4 times. It is a brutal protocol, but it is the fastest way to expand your stroke volume and lung capacity.

3. Speeding Up Your Step

  • Leg strength: Focus on basic lower-body movements like squats, lunges, and calf raises. Without strong quads, glutes, and calves, your body simply won’t have the mechanical power to push off the ground efficiently.
  • Interval walking: During your regular daily walk, consciously speed up to your absolute maximum pace for 1 minute (as if you are rushing to catch a train), then return to your normal pace for 2 minutes. Repeat this 5 times during your walk.
  • Balance and Core Stability: Planks, side planks, and standing on one leg with your eyes closed will strengthen your core and stabilizers. When your brain feels stable and balanced, it automatically allows you to take longer, faster, and more confident strides.
