How Photobiomodulation May Affect Health at the Cellular Level

To understand the potential benefits of red light therapy, it is helpful to first look at cellular respiration—the process through which cells produce energy. Cells are able to create and use energy that is required for all of the important physiological processes that keep a person in good health. A large portion of the energy for this is in the form of adenosine triphosphate, also known as ATP. Most ATP is produced inside cellular organelles known as mitochondria. In the aging process, mitochondria eventually stop functioning properly, and as they are no longer able to effectively produce energy, this leads to a breakdown of the cells, inflammation in the body, and faster aging.

Cytochrome c Oxidase and Its Role in Cellular Energy Production

The first way red light therapy helps to improve the function of the cells is the fact that a particular wavelength of red light or near-infrared light is absorbed by an enzyme called cytochrome c oxidase, which exists inside the mitochondrial respiratory chain of the cell. Under certain conditions, nitric oxide (NO) may bind to cytochrome c oxidase and temporarily interfere with mitochondrial respiration and oxygen utilisation. This ultimately inhibits ATP synthesis and leads to the buildup of reactive oxygen species (ROS). Red light boosts the synthesis of ATP, by freeing the NO from the enzyme in a cascade of molecular reactions:

  • Cytochrome c oxidase absorbs red/NIR, leading to NO dissociation from the complex.
  • Oxygen is then able to bind the enzyme freely.
  • Mitochondrial respiration may improve, potentially increasing ATP production.
  • Photobiomodulation briefly boosts ROS levels, leading to cellular adaptations, and thus, the cells become better prepared against future stressors (called hormesis).

By restoring the function of the mitochondria, cells are once again able to get the energy they need to repair damage, synthesize new proteins and expel waste.

  • Benefits may be cumulative. The effects are seen over multiple treatments (ex: most studies recommend 3–5 sessions a week for multiple weeks).

Hype:

  • More is better. Claims that high-powered red-light panels or longer exposure times automatically produce faster or better results.
  • Deep penetration. While NIR light can travel centimeters into skin tissue, bone and brain tissue, it cannot travel much past that.
  • “Full body cure-all”, Claims of healing any/all ailments (visceral, metabolic disorders etc.) with a single red light therapy home panel.
  • Instant results. Claims of reversal of aging, wrinkles and chronic pain from one exposure.

Is Red Light Therapy a Longevity Tool?

Red light therapy is being investigated as a potential supportive tool within the field of healthy aging. It’s not a cure-all, but it does target a root cause of aging (mitochondria dysfunction).

Photobiomodulation may support mitochondrial function and cellular energy production and may help modulate inflammation and tissue repair. It is generally considered non-invasive when used appropriately, although its effectiveness depends on the condition being treated, the device, the wavelength, the dose, and the treatment protocol.
